How Alcohol Affects Diabetes

Diabetes is a complicated medical condition that requires the patient to pay strict attention to their diet. It takes a huge committment for the patient, meaning that they must pay attention to the foods they eat, take medications and test their blood sugars regularly. Just by taking these simple steps it is possible to improve your blood sugar levels. Most patients are good about watching their diet but tend to forget that certain things, like alcohol intake, can cause a huge spike in blood sugars and can undo all of their hard work!

Alcohol is a beverage that is rich in calories and low in nutrition. It actually provides practically no nutritional value. Alcoholic beverages are broken down into simple sugars and glucose within the body, increasing blood sugar levels. Unfortunately, even a small amount of alcohol can raise blood sugar dramatically. Although patients with diabetes do not have to cut alcohol out completely, they should take care not to overindulge and remember that alcohol can have other effects on blood sugar.

Too much alcohol can cause low blood sugars, driving them to a dangerously low level. AIt is a good idea to have a glucometer such as the Bayer Contour glucose meter on hand to check sugars in case you start to feel weak, shaky or lightheaded. These are symptoms of hypoglycemia.

As previously discussed, alcohol can also cause high blood sugars that can exacerbate chronic complications of diabetes such as diabetic retinopathy, foot problems and kidney disease. It is important to monitor sugars carefully if you do drink. Ask your doctor for a free glucometer to help you track your blood sugars.

Alcohol can affect diabetes in several other ways. Drinking alcoholic beverages can also raise triglycerides, which can build up and cause fatty liver disease.In addition, alcohol can increase blood pressure readings and can interact with many medications.Drinking may also trigger the appetite and cause patients to overeat or to snack frequently.

Patients who drink too much alcohol increase the risk of complications from diabetes and raising overall sugar levels and the a1c blood test. High triglycerides contribute to heart disease and liver problems. In addition, when these levels are extremely high it can trigger acute pancreatitis, a dangerous condition that is life threatening.

If you have diabetes, talk with your doctor to see if it is okay for you to have an occasional glass of alcohol. If you are allowed to drink alcohol, remember not to overindulge because excessive amounts can cause significant diabetic complications.

    The medical profession states that “Diabetes is a chronic illness”, which creates a change in the body by reducing or removing the body’s capability to break down the glucose that is consumed within the food we eat. Glucose is used by the body as its source of power.

    About Insulin.

    The body produces Insulin, in the Pancreas. Insulin is a hormone used to centrally regulate the metabolism of fat and carbohydrates in the body. Insulin causes the cells in the Liver, Muscles and Fat tissue to soak up Glucose from the blood, or absorb it, storing it as “Glycogen”. Insulin is produced to remove excess glucose from the blood, which would otherwise become toxic. The Liver also regulates its secretion of Glucose in the presence of Insulin. When blood glucose levels fall below a certain level the stored sugar (in muscles and fat tissue) is used. This process of extracting the stored sugar is called “Glycogenolysis”. This means that for the body energy system to function properly, the creation of Insulin has to be in balance with the need to control Blood Glucose i.e. Glucose in the blood, as opposed to stored as Glycogen in the Liver, Muscles or Fat tissue.

    About Diabetes.

    Diabetes comes about when the body either cannot produce enough Insulin (Type 1) or the body develops a resistance to Insulin (Type 2) and subsequently the Insulin is unable to do its work in controlling the glucose levels in the blood or releasing store Glycogen for use as energy.

    • HypErglycemia is the condition when there is a higher than normally concentration of blood sugar.
    • HypOglycemia is the condition when there is a lower than normal concentration of blood sugar.

    Circulatory problems, especially heart disease, are the major cause of premature death in people with diabetes, and prevention of these problems is a central goal in the treatment of diabetes. You can take a number of steps to reduce your risk of heart disease and circulatory problems:

    Improve glucose control. The target levels are described.

    Lower blood pressure. I have already discussed in the “Diabetic Kidney Disease” section earlier in this chapter the importance of controlling blood pressure. The treatment goal is to get the systolic blood pressure (top number) below 130 mm Hg and the diastolic blood pressure (bottom number) below 80.

    Take aspirin. Clinical studies show that aspirin reduces the risk of heart attacks and stroke. Aspirin works by making platelets (the cells in the blood that are responsible for blood clotting) less sticky. Recent studies indicate that lower doses are just as effective as higher doses, so low-dose aspirin therapy (a 75-81 mg dose once a day) is recommended if you already have heart disease or circulatory problems. If you cannot tolerate aspirin because of allergic reaction or another reason, your doctor may prescribe a medicine called clopidogrel (Plavix), which also makes the platelets less sticky. Individuals with type 1 and 2 diabetes without heart disease or circulatory problems should also take low-dose aspirin therapy if they have a strong family history of heart disease; if they have kidney damage, high blood pressure, or lipid problems; if they are smokers; or if they are older than forty years.

    Stop smoking. Smoking increases the risk of both macrovascular and microvascular damage in people with diabetes.

    Improve your lipid profile. People with type 2 diabetes frequently have abnormalities of their lipid profile-a collective term for cholesterol, triglycerides, HDL cholesterol, and LDL cholesterol levels. Almost all people with type 2 diabetes and many with type 1 diabetes need to take medicines for the lipid abnormalities.

    In people with diabetes, the levels of triglycerides are frequently too high and the levels of HDL are too low. In addition, people with diabetes tend to have a form of LDL particles called small, dense LDL, which can abnormally collect in the blood vessel walls and cause atherosclerosis. Research has shown that correcting these lipid abnormalities in people with diabetes reduces the development of atherosclerosis. The goals of treatment are to

    Lower the LDL cholesterol to at least below 100 mg/dl but ideally to 60 to 70 mg/dl

    Lower the triglyceride level to below 150 mg/dl

    Raise the HDL cholesterol to more than 40 mg/dl in men, and to more than 50 mg/dl in women

    Complications of the Diabetic Cardiovascular System

    The large blood vessels in people with diabetes undergo changes due to atherosclerosis; abnormalities in platelets, red blood cells, and clotting factors, and changes in arterial walls. It has been shown through research that atherosclerosis has an increased incidence and an earlier age of onset in people with diabetes, although the reasons for this are unknown.

    Other risk factors that contribute to the development of macrovascular disease of diabetes are hypertension, hyperlipidemia, cigarette smoking, and obesity. Alterations in the vascular system increase the risk of the long term complications of coronary artery disease, cerebral vascular disease, and peripheral vascular disease.

    Alterations in the small blood vessels in people with diabetes involve structural defects in the basement membrane of smaller blood vessels and capillaries. The basement membrane is the structure that supports and serves as the boundary around the space occupied by epithelial cells. These defects cause the capillary basement membrane to thicken, eventually resulting in decreased tissue perfusion.

    Changes in basement membranes are believed to be due to one or more of the following: the presence of increased amounts of sorbitol (a substance formed as an intermediate step in the conversion of glucose to fructose), the formation of abnormal glycoproteins, or problems in the release of oxygen from hemoglobin. The affects of these alterations affect all body tissues but are seen primarily in the eyes and kidneys.

    Coronary Artery Disease:

    Coronary artery disease is a major risk factor in the development of myocardial infarction in people with diabetes, especially in the middle to older adult with type 2 diabetes. Coronary artery disease is the most common cause of death in people with diabetes, accounting for 40 to 60 percent of all cases of mortality. Diabetics who have myocardial infarction are more prone to develop congestive heart failure as a complication of the infarction and are also less likely to survive in the period immediately following the infarction.

    High Blood Pressure (Hypertension):

    High blood pressure is a common complication of diabetes. It affects 20 to 60 percent of all people with diabetes, and is a major risk factor for cardiovascular disease and microvascular complications such as retinopathy and nephropathy. Hypertension may be reduced by weight loss, exercise, and decreasing sodium intake and alcohol consumption. If these methods are not effective, treatment with high blood pressure drugs is necessary.

    Stroke:

    Diabetics, especially older adults with type 2 diabetes, are 2 to 6 times more likely to have a stroke. Although the exact relationship between diabetes and cerebral vascular disease is unknown, high blood pressure, which is a risk factor for a stroke, is a common health problem in those with diabetes. I addition, atherosclerosis of the cerebral vessels develops at an earlier age and is more extensive in people with diabetes.

    The signs and symptoms of impaired cerebral circulation are often similar to those of hypoglycemia and include blurred vision, slurred speech, weakness, and dizziness. Diabetics with these symptoms have potentially life threatening health problems and require constant medical attention.

    Peripheral Vascular Disease:

    Peripheral vascular disease of the lower extremities accompanies both types of diabetes, but the incidence is greater in people with type 2 diabetes. Atherosclerosis of vessels in the legs of diabetics begins at an early age, advances more rapidly, and is common in both men and women. Impaired peripheral vascular circulation leads to peripheral vascular insufficiency with intermittent pain in the lower legs and ulcerations of the feet. Occlusion and thrombosis of large vessels and small arteries and arterioles, as well as alterations in neurologic function and infection, result in gangrene.

    Gangrene in diabetics is the most common cause of non-traumatic amputations of the lower leg. In people with diabetes, dry gangrene is the most common, and is manifested by cold, dry, shriveled, and black tissues of the toes and feet. The gangrene usually begins in the toes and moves proximally into the feet.

    The Diabetic Exercise Program

    An important part of any diabetic management program is regular exercise. The benefits of exercise are the same for everyone, whether they have diabetes or not. Improved physical fitness, improved emotional state, weight control and improved work capacity are all benefits of exercise.

    Diabetics exercise increases the uptake of glucose by muscle cells, potentially reducing the need for insulin. Exercise also reduces cholesterol and triglycerides, reducing the risk of cardiovascular disorders. People with diabetes should consult their primary health provider before beginning or changing an exercise program.

    The ability to maintain an exercise program is affected by many different factors, including fatigue and glucose levels. It is as important to assess the diabetic’s usual lifestyle before establishing an exercise program as it is before planning a diet. Factors to consider include the diabetics usual exercise habits, living environment, and community programs. The exercise that the person enjoys most is probably the one that he or she will continue throughout life.

    Everyone with diabetes should follow the guidelines set forth by the ADA when undertaking an exercise program. These include the use of proper footwear, inspecting the feet daily and after exercise, avoiding exercise in extreme heat or cold, and avoid exercise during periods of poor glucose control. The ADA further recommends that people over the age of 35 have an exercise-stress electrocardiogram prior to beginning an exercise program.

    Exercise for Type 1 Diabetics.

    In the person with type 1 diabetes, glycemic responses during exercise vary according to the type, intensity, and duration of the exercise. Other factors that influence responses include the timing of exercise in relation to meals and insulin injections, and the time of day of the activity. Unless these factors are integrated into the exercise program, the person with type 1 diabetes has an increased risk of hypoglycemia and hyperglycemia. The following are some general guidelines for an exercise program.

    People who have frequent hyperglycemia or hypoglycemia should avoid prolonged exercise until glucose control improves.

    The risk of exercise-induced hypoglycemia is lowest before breakfast, when free insulin levels tend to be lower than they are before meals later in the day or at bedtime.

    Low-impact aerobic exercises are encouraged.

    Exercise should be moderate and regular; brief, intense exercise tends to cause mild hyperglycemia, and prolonged exercise can lead to hypoglycemia.

    Exercising at a peak insulin action time may lead to hypoglycemia.

    Self monitoring of blood glucose levels is essential both before and after exercise.

    Food intake may need to be increased to compensate for the activity.

    Fluid intake, especially water, is essential.

    Young adults may continue participating in sports with some modifications in diet and insulin dosage. Athletes should begin training slowly, extend activity over a prolonged period, take a carbohydrate source such as an energy drink after about one hour of exercise, and monitor blood glucose levels for possible adjustments.

    In addition a snack should be available after the activity is completed. It may be necessary to omit the usual regular insulin dose prior to an athletic event; even if the athlete is hyperglycemic at the beginning of the event, blood glucose levels will fall to normal after the first 60 to 90 minutes of exercise.

    Exercise for Type 2 Diabetics.

    An exercise program for the type 2 diabetic is especially different. The benefits of regular exercise include weight loss in those who are overweight, improved glycemic control, increased well being, socialization with others, and a reduction of cardiovascular risk factors.

    A combination of diet, exercise, and weight loss often decreases the need for oral hypoglycemic medications. This decrease is due to an increased sensitivity to insulin, increased caloric expenditure, and increased self esteem. In fact regular exercise may prevent type 2 diabetes in those at high risk for getting this form of diabetes.

    Here are some guidelines for type 2 diabetics undertaking an exercise program.

    Before beginning the program, have a medical screening for previously undiagnosed hypertension, neuropathy, retinopathy, nephropathy, and cardiac ischemia.

    Begin the program with mild exercises and gradually increase intensity and duration.

    Self monitor blood glucose before and after exercise.

    Exercise at least three times a week or every other day, for at least 20 to 30 minutes.

    Include muscle-strengthening and low-impact aerobic exercises in the program.

    Diet, medication and exercise are all an important part of a successful program to manage diabetes. It is important for any diabetic to incorporate all three into their lives to control and prevent the many complications that this disease can bring.     If you are overweight and have type II Diabetes, you’re not alone. In fact, more than ten percent of overweight Americans are among the twenty million men and women suffering with this debilitating disease.

    Do you have diabetes? Diabetes is a condition where the pancreas is unable to manufacture enough insulin in order to process the glucose that makes its way into the bloodstream. Since glucose is a form of sugar, diabetics are typically cautioned to refrain from eating foods that are made with sugar.

    However, your body needs glucose for energy. The insulin shots that diabetics have to give themselves provides the body with the ability to break down the glucose. However, if you don’t use up all the glucose that you eat, your body turns it into fat and deposits it all over the body and reserves it for use later.

    Thankfully, a lot of diabetics have been able to get their diabetes under control by quitting smoking, avoiding alcoholic drinks, eating a sensible diet and exercising daily to lose weight. A word of caution, it is a good idea to have a discussion with your medical practitioner before making any modifications to your diet and/or exercise regimen. Below are three methods for losing weight:

    1. Drinking water is cool. Water not only helps prevent dehydration, but it also helps in the digestive function of foods and takes away the harmful toxins in your system. Also, small amounts of unsweetened tea and/or coffee may be beneficial.

    2. Eat a balanced low-fat diet. Your diet should consist of lean proteins and small amounts of carbohydrates and fats. Avoid foods containing white bread, pasta, french fries and white rice. These foods are loaded with carbohydrates and processed sugars that transform into stored fat unless you will be running in a marathon and will use up all of those calories. Stick with whole grains like brown rice and small portions of wheat pasta. Also, small quantities of red grapes can give you an antioxidant nutrient boost.

    3. Create a daily exercise routine. Studies have determined that physical workouts on a regular basis increases heart and circulatory health. When your heart pumps blood and it circulates through the body, exercising keeps it moving. When your blood sits still due to a lack of exercising, for instance, fat deposits can build up and harden causing the blood to clot which can result in heart disease and strokes. If you are able to walk for a few minutes each day it can have a positive change in your health and fitness.

    As this article can attest, these weight loss tips may help reduce and potentially even eliminate your reliance upon insulin shots. Additionally, you will have an increased chance of living a happier, healthier way of life. Even though there is no cure for diabetes, it doesn’t have to be a curse either. Take it one tip at a time and discuss it with your health care professional.
