    Contrave, the investigational diet pill combination of naltrexone and bupropion, has been granted a possible new lease on life

The company said it hoped to initiate such a trial in the first half of 2012 after discussions with the FDA.

    Writing in Forbes , Matt Herper pointed out that even if the trial is successful approval is far from guaranteed:

    The FDA told Orexigen, in writing, that “if the interim analysis meets the specified criteria to exclude an unacceptable increased cardiovascular (CV) risk, the drug could be approved.” That “could” is pretty important ” the FDA might consider the results of such a trial and decide that Orexigen’s risks still don’t outweigh its benefits.

    Sanjay Kaul, who was a member of the Contrave advisory panel, sent this comment:

    It is heartening to hear the FDA is moving in the right directionwith regards to establishing a regulatory standard for approval ofobesity drugs that is modeled after the diabetes guidance.     A great choice is usually to carry a packed lunch each day instead. This can save you cash and in reality doesn’t require a great deal time to make in the morning. Purchase a nutritious lunch meat and low- fat cheese combination that you enjoy, or get hold of ready- to- go salad that you’ll bring with you to work.

    Diet Plans | Approach A Portion With Caution

    AMANDA says: “Portions have crept up and up over the past few decades - but we can look to the Seventies for inspiration and cut calories while still enjoying the foods we love.

    “In the 1970s, an average packet of crisps was 25g. Now they are often double this. How many people do you know who, on picking up a 50g bag, say, ‘Crisps used to come in 25g bags, so I’ll just eat half’?

    “It doesn’t happen and you end up eating 268 calories instead of just 134.

    “This guide will help you become more aware of serving sizes - and will, hopefully, help you shed any excess weight.”

    BREAKFAST CEREALS It isn’t just that portions are getting bigger. Most of us find it hard to know what a serving recommended on packs of food actually looks like.

    Breakfast cereals are a classic example. To know for sure, you need to get the scales out and start weighing.

    Fruit and fibre: An average serving is 40g = 143 calories. A small 30g bowl has 107 calories. A larger 50g bowl which most of us would naturally pour out provides 177 calories. The same goes for cornflakes and bran flakes.

    Porridge oats: Individual packs of Oat So Simple oats weigh just 27g = 98 calories. When made up with 180ml of skimmed milk this = 161 calories. A typical home-made portion of 45g of oats = 160 calories. Made up with 340ml of skimmed milk = 279 calories per serving. A 300g serving of ready-made porridge, including milk, at a High Street coffee shop = 242 calories.

    SCONES An average 1970s scone weighed 48g. Today a typical scone in a bakery or supermarket four-pack is likely to be 72g = 227 calories. Beware monster-sized scones in coffee shops. I found one which weighed 190g = 600 calories. You could have six bananas for this.

    MUFFINS These vary hugely. A mini muffin in a supermarket multipack is 30g = 115 calories. A muffin in a typical four-pack in supermarkets is 114g = 438 calories. A muffin in a typical coffee shop is 128g = 492 calories. And whether blueberry, carrot or banana, they are still packed with sugar and fat.

    BREAD Slices vary greatly in size - one from a large thick-cut loaf has almost double the calories of a small white sliced version. A small white loaf is 27g per slice = 61 calories. A medium-sliced loaf is 35g per slice = 79 calories. A thick-sliced loaf is 50g per slice = 113 calories.

    PITTA These too vary greatly in size and calories. A mini pitta is 35g = 89 calories. An average one is 75g = 191 calories and a large one is 95g = 242 calories.

    SALMON A 1970s serving was 83g = 163 calories. Today salmon fillets are around 110g = 217 calories. A salmon steak is about 160g = 311 calories.

    CHICKEN BREAST A 100g serving of lean chicken breast = 128 calories. It is worth weighing different size chicken breasts at home to get an idea of what a 100g portion looks like.

    LEAN SLICED BEEF An average 35g slice of roast beef = 50 calories. Thick slices can be double this.

    SAUSAGE ROLLS These can be loaded with calories and fat. A mini buffet-style roll is 16g = 61 calories. A supermarket multi-pack roll is 34g = 130 calories. A typical bakery roll is 66g = 251 calories. In filling stations and independent supermarkets, they can weigh 144g = 549 calories.

    BAKED POTATO An average size is 180g = 245 calories. Weigh your potatoes before you bake them and opt for smaller ones from takeaways.

    CHIPS In the 1970s an average portion was 160g = 382 calories. Now a chippy portion is 240g = 574 calories. Try to stick with around ten chips as a portion.

    PASTA A 1970s serving of dried spaghetti averaged 70g = 239 calories. Today servings are more like 150g = 512 calories, without sauce.

    NO time to exercise? You could squeeze in more than an hour a day and burn around 400 calories without even putting your trainers on. Just follow Nicki’s bite-size workouts.

    7am - THE POWER SHOWER: Tones and strengthens your chest, shoulders and arms. Use your lather time to blast your muscles awake

    7.30am - BREAKFAST BLITZ: Tones and strengthens legs, bottom and thighs. While waiting for your porridge to cook, hone lithe legs with alternate lunges in front of the stove. CALORIES BURNED: 30 in 3mins.

    8am - COMMUTING CORE CONDITIONING: Tones tummy muscles. Next time you’re stuck in traffic, suck that belly in and hold for ten seconds at a time. Can be done standing or sitting on the bus or train too. CALORIES BURNED: 25 in 6mins.

    11am - BEAT THE SLUMP: Tones inner thighs. Tackle morning energy slumps

    1pm - LUNCHTIME WALK: Great cardio workout. Try power-walking to a cafe or shop that’s at least ten minutes away. Then run up the stairs two at a time - don’t wait for the lift. CALORIES BURNED: 150 in 20mins.

    4pm - PHOTOCOPY CLENCHES: Tones and strengthens your buttock muscles. Make the wait at the photocopier work for you - clench your bum muscles and hold for a count of ten. CALORIES BURNED: Ten in 3mins.

    8pm - SOFA SCULPTING: Tones and strengthens the backs of your arms. Slide your bum forwards and lift yourself off the seat with your weight on your hands. Bend and straighten arms to lower your bum below seat level, lift up and down. CALORIES BURNED: 30 in 3mins.

    9pm - FITNESS ON THE PHONE: Tones and strengthens your core muscles. Making calls is a great time to strengthen your middle. Sit on a Swiss ball and move your bum around on it as you talk. CALORIES BURNED: 30 in 10mins.

    10pm - SQUAT AND BRUSH: Tones and strengthens your legs, bottom and thighs. Stand with feet wider than shoulder width apart, toes out to 45 and squat. Pearly whites and buns of steel in no time. CALORIES BURNED: 20 in 3mins.

    Mindy Moss’s customers are shocked when they see her. They want to know how the well-known Winnipegger got her newly svelte figure, which seemed to appear overnight.

    The answer? It happens every morning at Eyelet Dove, her popular Academy Road lingerie boutique. There, amongst the clutter of her tiny shop kitchen, she injects herself–usually in the abdomen– with human chorionic gonadotropin, also known as HCG.

    It has been her ritual since May when she signed up for the HCG diet, administered

    Moss has lost more than 50 pounds on the diet — 10 in the first week alone. She’s got 20 more to go.

    The controversial weight-loss plan, invented in the 1950s, involves consuming just 500 calories a day and injecting HCG, a hormone that women produce during pregnancy. The hormone helps the fetus get necessary nutrients. Men also produce the hormone, but in smaller amounts.

    Health Canada first approved prescription HCG in the 1980s — in its injectable form — for women undergoing fertility treatment.

    Chubaty says HCG, when injected for weight loss, suppresses appetite and prevents the dangerous “metabolic collapse” associated with ultra-low-calorie eating.

    Moss, who has struggled with her weight for most of her 57 years, is proud of her new body.

    “Would you like to see me do a one-armed pushup?” asks the vivacious Moss during an interview in her chic boutique, which is painted a soft pink with black accents.

    The Tuxedo grandmother unzips her Lululemon jacket and peels it off to reveal her bare arms. She flexes them, showing off a muscular bulge — an attempt to prove she’s fitter than ever and even has enough energy to sustain twice-weekly workouts with her personal trainer.

    Moss is aware that her new diet has critics, who say that injecting a hormone is an unnecessarily extreme measure to take for weight loss.

     

    They worry that 500 calories doesn’t offer the vitamins and minerals healthy bodies need to stay healthy.

    Others question the safety of injecting HCG over a long period of time (the hormone has been linked to certain hormone-driven cancers).

    Some medical professionals even doubt whether HCG has any effect on the appetite at all. They suggest that anyone ingesting a mere 500 calories a day will lose weight. A Harvard physician on a recent episode of the Dr. Oz television show said 14 randomized controlled trials showed that participants given placebo lost just as much weight as participants taking HCG.

    On that same Dr. Oz episode, one woman shared her bad experiences with the HCG Diet — she fainted during her second week on the regimen, and her menstrual cycle stopped for four months after she quit.

    Moss, who has recently upped her calorie intake to 750 and still continues to lose weight, doesn’t care what the critics say.

    “I don’t know why it works. I don’t know how it works. I just know it works,” says Moss, who didn’t tell her friends, kids or husband when she first signed up for the diet.

    Within a few days on it, she says her once-ravenous appetite (she could down a whole cheesecake and a whole loaf of bread) was soon satisfied with a few meagre ounces of protein and a vegetable for lunch and dinner.

    “This drug has given me the power to say no, for whatever reason. I’m not a medical doctor. I don’t get it,” says Moss, who got over her fear of needles shortly after giving herself her first injection.

    Chubaty, a family physician, brought the diet to the city a few months ago. The Winnipeg doctor — who also practises in Scottsdale, Ariz. — says over the past five years, he’s administered HCG for weight loss to more than 4,000 patients in the United States.

    He says none have had serious side effects.

    Here in Winnipeg, about 10 patients follow the diet under his care. (He administers the diet out of an office at Tache Pharmacy — separate, he says, from his family practice).

    He also offers oral HCG drops for patients who are afraid of needles, though he says injections provide the “best bang for your buck” considering they are the same price.

    According to Health Canada, oral HCG has not been “authorized” in Canada.

    HCG — even in its injectable form — is not covered under insurance plans.

    Chubaty charges patients $400 for a six-week diet plan. The price drops to $200 for repeated cycles after that.

    In addition, the prescription costs $100 for an eight-week supply.

    Chubaty grew up in Winnipeg’s North End, moved to Arizona in the 1980s and recently came back to his hometown to practise part time. He says he’s selective about who he allows to go on his diet

    He says patients should be cancer-free for at least 10 years before they try the HCG Diet. For patients who have less than 30 pounds to lose, “we try to steer them away from it.”

    He also warns that patients need to disclose “underlying” problems such as migraine headaches or colitis, because the HCG Diet may exacerbate such ailments. Nevertheless, he says “that’s very rare.”

    Chubaty says the minor side effects a healthy patient may feel on the weight-loss plan are worth the results.

    “If there’s any slight chance that you’re going to be off a little bit, you’re going to be much healthier six to 12 weeks from now, when you shed 50 pounds, than you were worrying about your nutrition,” he says during a phone interview.

    “People come in overweight because they are over-nutritioned, not under-nutritioned.”

    He’s aware of his critics.

    “Whenever anything new comes out, there are always going to be the naysayers in medicine,” says Chubaty. “You’ve got to be quite outside the box. It’s there. It’s safe.

    “The worst thing that can happen is you’re going to waste your money and not lose any weight.”

    Health Canada spokeman Gary Holub says using HCG injections for anything other than fertility treatment would be considered “off-label” use.

    He says Health Canada has a number of concerns about using HCG as a diet aid.

    In an email to the Free Press , Holub writes, “When used to treat a condition for which it is not indicated, such as weight loss, Human Chorionic Gonatrophin could cause a number of serious side-effects. In women, this could include painful cysts occurring as the result of over-stimulation of the ovaries.”

    Holub also says that each province’s College of Physicians and Surgeons regulates the off-label use of drugs, not Health Canada.

    Dr. Anna Ziomek, assistant registrar of Manitoba’s College of Physicians and Surgeons, says that her organization was not aware of the HCG Diet or that a local physician is administering it.

    She says physicians do not have to approve the off-label use of drugs with the College.

    “What we expect physicians to do is to have the skilled knowledge and judgment to do the things that they are doing,” says Ziomek, who practises emergency medicine.

    “Our position on alternative therapy is that if physicians want to participate in that, it’s fine to do so as long as a patient is understanding that this isn’t the usual use,” says Ziomek.

    She says she doubts the diet would work in the long term because most people would find the low-calorie regimen too difficult to maintain over the long haul.

    “I don’t think there would be many dietitians or nutritionists who would support the patient doing this with the long-term goal of losing weight and maintaining a healthier weight.”

    Moss feels that her situation proves otherwise.

    She skipped a few doses of HCG last weekend during a trip to Minneapolis and came back a couple of pounds lighter.

    Her eyes well up with tears as she recalls the moment — shortly before she went on the HCG Diet — when she tried on a beautiful nightie one of her best lingerie suppliers gave her as a gift.

    She didn’t like what she saw.

    “I looked at myself, and I was hideous,” says Moss. “I’ve never felt lower than that point. I couldn’t accept that I looked this bad in product that I sell all the time.”

    Today, Moss loves what she sees in the mirror when she slips on the same nightgown.

    “I look great in it. It does everything it’s supposed to do, and more.”

    Three months after Orexigen Therapeutics shelved further development plans for its Contrave diet pill because the FDA made an “unprecedented” request for additional clinical trial work, the aspiring drug developer has reached an unexpected deal with the agency to proceed with a cardiovascular study.

    At issue are concerns over cardiovascular risks that prompted the FDA to request a randomized, double-blind, placebo-controlled trial, which Orexigen execs claimed “would generate significantly more information than is necessary or feasible.”Now, though the FDA agreed to a study involving 10,000 patients and would be completed and reviewed

    “I think this was a multi-pronged effort that we built to create some momentum that helped the process,” Orexigen ceo Michael Narachi told analysts on a conference call last night. “…This drug, which we always believed could benefit patients, has a clear path to market” (see a statement here ).

    The disclosure comes just one week after the FDA agreed to reconsider yet another diet pill. Vivus, which is developing the Qnexa weight-loss drug, will pursue an indication for men and women who do not have the potential to have children, and will resubmit data

    The moves suggest that there may – emphasis on ‘may’ – be a future after all for this latest round of forthcoming diet pills, each of which has suffered setbacks trying to overcome regulatory hurdles. Last year, the FDA also rejected the Lorqess drug from Arena Pharmaceuticals due, in part, to concerns the pill may cause tumors in rats and that efficacy was marginal ( back story ).

    As Leerink Swann analyst Joshua Schimmer wrote in an investor note, “With over 70 million Americans now defined as obese, weight loss has become a major unmet medical need and is a major contributor to significant conditions such as diabetes and cardiovascular disease.” This helps explain why the FDA plans to hold an advisory committing meeting early next year to review diet pills.

    However, the FDA has been particularly cautious about approving new diet pills. The backdrop, of course, is greater agency emphasis on safety and, in particular, the 1997 withdrawal of Pondimin, which was one-half of the infamous fen-phen weight-loss cocktail and its chemical cousin, Redux, over links to serious heart and lung side effects.

    The agency is also concerned that diet pills may be prescribed and used inappropriately. The fen-phen craze spawned a number of so-called pill mills and FDA officials fear that many consumers will rely too heavily on diet drugs without simultaneously altering their diet or exercising more.
